Job Description :

The RN – Adult Care provides comprehensive nursing care to adult patients in a home health setting within the Bronx Borough. Responsibilities include assessment, planning, implementation, and evaluation of patient care in accordance with established standards and regulatory requirements. The role requires traveling to patients’ homes to provide care for a range of cases, typically 5–8 visits per day.

Key Duties :

  • Deliver home health nursing care to patients aged 18 to 100 years.
  • Perform comprehensive patient assessments and develop care plans.
  • Administer medications and treatments as prescribed.
  • Coordinate with interdisciplinary healthcare teams to ensure continuity of care.
  • Document patient progress and maintain accurate records in compliance with policies.

    • Required Skills & Experience :

  • Minimum of 2 years of acute care experience (Med-Surg, Maternal / Child Care, Critical Care, or Pediatrics).
  • Current adult home health experience required.

    • Education :

  • Required : Associate Degree in Nursing (ASN / ADN) from an accredited program.
  • Preferred : Bachelor’s Degree (BSN).

    • Certifications & Licensure :

  • Active New York State RN License.
  • Primary Source Verification from NYSED.
  • AHA BLS Certification.

    • Weekly Breakdown : \t\t\nWeekly Gross : \t $3,850.00 \t Per Week \n
