Must-Have :

• Experience in inpatient and outpatient treatment settings
• Experience in group therapy focused on treating substance use disorders
• Knowledge of psychoeducation, 12 Step Recovery philosophy, and relapse prevention

Description :

This position involves providing psychosocial assessment and therapy for patients and their families in an outpatient setting.

The role includes discharge and aftercare planning for individuals receiving treatment for substance use disorders.

Services delivered will be based on recovery-oriented practices, family systems, and evidence-based treatments.

The LCSW will serve as the primary contact for involving and educating patients, families, and significant others in the treatment and mental health recovery process.

Collaboration with community programs to develop strategic partnerships will be essential for enhancing aftercare plans for patients and families.

This position requires performing assessments and developing treatment plans to meet patient and caregiver needs.

The LCSW will document care delivery and patient responses according to the plan of care.

The individual will provide crisis intervention, family therapy, individual counseling, and education for patients and caregivers.

Knowledge of substance use disorders, psychiatric disorders, and appropriate treatment options is necessary.

The role includes working with a multidisciplinary team to ensure continuity of care.
