Washington RN Case Manager

A company is looking for an RN Case Manager.

Key Responsibilities

Ensure medically appropriate transitions of care for patients and families

Advocate for patients during transitions to lower levels of care

Collaborate with healthcare professionals in patient care activities and care transitions

Required Qualifications

Bachelor's degree in Nursing or obtained within five years from the initial date of hire

Current Registered Nurse license in Washington State or Multistate License endorsement

Four years recent hospital case management experience

Five years of recent clinical hospital / ambulatory experience

Advanced degree in Nursing preferred
