Description :

RN Case Manager needed for home hospice care.

The position involves managing symptoms and providing support to patients during their end-of-life journey.

Visits will be conducted in patients' homes, and various facilities such as private residences and assisted living centers.

The daily case load includes five points per day, with specific weights assigned to different visit types.

Revisits count as one point, while admissions count as 2.75 points.

Interdisciplinary team meetings are held weekly to ensure comprehensive patient care.

Candidates must have expertise in patient care, symptom management, and coordination with interdisciplinary teams.

EPIC charting experience is required for this position.

Candidates should have strong communication skills and the ability to work independently.

Floating may be requested based on facility needs.

All candidates should have specialty experience within the last year.
