Licensed Practical Nurse (FHC)

CHARACTERISTICS OF THE WORK\n\nUnder supervision, performs nursing work.\n\nResponsible for assisting providers and delivering nursing care in a primary care center under the auspices of the Family Health Centers (FHC).\nEXAMPLES OF THE WORK (These statements are intended to be illustrative of the work of this class.)\n\nDocuments patient history and assesses chief complaints.\nReviews charts and makes entries.\nPrepares patient and room for appropriate examination procedures.\nAssists providers with exams and procedures.\nAdministers medications, injections, immunizations, and venipunctures as ordered by physician.\nHandles all abnormal and normal laboratory test results according to protocol.\nProvides individual and group health education instruction and information.\nMentors and acts as a resource for health services students and departmental personnel.\nProvides appropriate patient care based on age-appropriate growth and development principles.\nAssists in triage, handles patient inquiries, schedules appointments, and assesses patient complaints via telephone and recommends appropriate action, according to prescribed protocol.\nPerforms nursing assessments and develops health care plans for patients.\nPerforms basic laboratory procedures.\nMakes referrals to the Registered Nurse (RN), supervisor, physician, or Advanced Practice Registered Nurse (APRN) for situations requiring further intervention.\nCleans, stocks, and orders supplies for the examination rooms.\nCleans and wraps instruments for sterilization.\nTravels between satellite clinics.\nAdheres to standard precautions.\nPerforms related work as required.\n\nMINIMUM REQUIREMENTS\n\nPossession of a current Licensed Practical Nurse (LPN) license issued by the Kentucky Board of Nursing OR Possession of a work permit to practice professional nursing in the State of Kentucky by endorsement.\n\nPHYSICAL REQUIREMENTS AND WORKING CONDITIONS\n\nMoves and / or lifts more than 100 lbs. when positioning patients or when assisting them on and off the examination table.\n\nIdentifies medical record documentation, requests and / or releases.\n\nResponds to patient and FHC needs via telephone, internet, fax, computer software and in person.\n\nSPECIAL REQUIREMENTS\n\nMust obtain CPR certification by the end of the probationary period and maintain certification throughout employment with FHC.\n\nMust provide own transportation to work at other FHC locations as needed.\n\nMust be available to work a varied schedule as needed.\n\nMust wear personal protective equipment (PPE) as mandated by OSHA and / or FHC.\n\nMust be vaccinated for Hepatitis B, have provided evidence of immunization, or have signed a declination form within 10 days of initial assignment.\nAn Equal Opportunity Employer\n\nTest Weights\nTraining & Experience : 100%\n\nPlease note that Training & Experience scores are based on the relevant (as determined by the hiring agency and Civil Service) training and experience, as listed on your application and / or resume, as submitted here.
