The Triage Nurse (LPN / RN) is key in promoting high quality, patient-centered care. She / he is responsible for assisting in patient management; proper documentation of medical transactions, managing triage calls in a consistent and timely manner : partnering with patient / family and pediatric care team on population management : and assisting in the daily operations of the office. The Triage Nurse is expected to maintain open, effective communication with the physicians, practice managers, practice executives, co-workers, patients, and outside vendors / agencies. She / he is expected to know and adhere to the philosophy and the Mission, Vision, and Values of PriMed.

Essential Duties and Tasks for the Triage Nurse (LPN / RN)
Procedures and Tasks :
- Assist in patient management, including but not limited to scheduling tests / procedures and preparing referrals / obtaining authorization as required by insurance, reviewing test results and communicating results to physicians and patients, and maintaining patient schedules.
- Manage clinical phone calls in a timely manner, including, but not limited to, prescription refill requests, referral requests, relaying physician messages, and patient call requests.
- Proper documentation according to policy guidelines.
- Maintain appropriate clinical tracking logs; lab specimens / results, immunizations, controlled substances, and any other logs as required.
- Maintain and safeguard the confidentiality of all patient data as well as other pertinent information acquired during the performance of her / his duties.
- Work all tasks lists, outstanding orders, referrals, etc. as assigned in E H R in a timely manner.
Coordination of Care
Timely completion of documentation to facilitate accurate patient summary information.Follows all administrative policies, clinical procedures, office processes, protocols, and standing orders as they exist to provide consistency in patient care.Anticipates patient needs based on disease processes and coordinates care accordingly.Coordinates patient's care between facilities and referral sources, ensuring that appropriate paperwork and medical information was sent to the referral source as well as tracking that consultation reports, etc. to ensure that they are received back and placed in the patient's chart.Reviews outstanding orders during pre-visit review and obtains critical data from referral sources :
Obtains lab and diagnostic tests.Obtains specialty office consult reports on all referred patients.Works overdue orders “work list” and identifies gaps in patient care. Report these gaps to the appropriate care team for follow-up.Proactively contacts populations of patients identified with specific health requirements for care based on their conditions, including monitoring of patients with missed appointments.Contacts patients and coordinates care when follow-up is needed.Assesses the patient's readiness for change and works with care team to provide appropriate referral / follow-up.Appropriately facilitates patient information among care team members to assure all patient care issues are addressed.Triage Nurse Communication
Maintains a positive attitude and supports a teamwork approach with co-workers, physicians, and all members of the care team.Identifies self to patients on the phone, wears name tags (Do we really need this?)Communicates and documents patient care need via the electronic health record (EHR) tool.Thoroughly reviews all EHR-based patient communication to ensure that all needs / follow-up items are addressed.Supports teamwork processes by communicating patient needs among care team members.Stays abreast of changes / updates in internal policies and patient care processes by completing required training and educational sessions, e-learning videos, and reading newsletters, etc.Recognizes gaps / opportunities for improvement of patient care and communicates to other care team members.Process Improvement
Contributes to development and improvement in organizational processes to enhance care of the patient.Identifies gaps or issues with administrative and patient care processes and notifies practice manager when re-evaluations need to be considered.Triage Nurse Qualifications, Education and Experience
Graduate of RN or LPN program with current licensure as required in the state of Ohio.Current certification in CPR.Minimum of one-year experience as Triage Nurse is preferred.Experience with EHR systemsThe Nurse (LPN / RN) physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
