Position Summary The RN Case Manager supports physicians and the interdisciplinary team in coordinating patient care with the goal of improving clinical outcomes, patient satisfaction, and cost efficiency. This role integrates utilization management, patient education, care facilitation, and discharge planning. Key Responsibilities Facilitate precertification and payor authorization Coordinate collaborative patient care across the continuum Apply process improvement methodologies to evaluate care outcomes Support clinical documentation and serve as a resource for coders Communicate effectively with physicians Provide age-appropriate care and understand life cycle principles Required Skills & Experience Strong interpersonal, communication, and negotiation skills Analytical and data management capabilities Proficient in PC and organizational / time management Ability to work independently and exercise sound judgment Preferred Qualifications Minimum of 3 years RN clinical experience Knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement Education & Licensure Required : Bachelor of Science in Nursing (BSN) Required : Active RN license in the State of Michigan Certifications : BLS (American Heart Association) Additional Requirements Minimum 5 years of acute inpatient case management experience Experience in acute discharge planning Familiarity with CarePort system Preferred experience with AllScripts, GEMS / Sunrise Must be comfortable working directly with patients and acclimating quickly By providing your phone number, you consent to : (1) receive automated text messages and calls from the Judge Group, Inc. and its affiliates (collectively "Judge") to such phone number regarding job opportunities, your job application, and for other related purposes.
