Oversee daily team operations, ensuring program goals and patient care standards are consistently met. Provide clinical oversight, support escalations, and manage a small panel of patients. Track and analyze team metrics including care minutes, call completion, and quality outcomes using Excel and software dashboards. Collaborate with cross-functional teams (clinical, enrollment, billing) to ensure smooth patient transitions and care coordination. Ensure all care delivery complies with clinical guidelines, documentation standards, and CMS program requirements. Active Nursing License (CNA, RN or LPN / LVN) 25 years of clinical experience , ideally in geriatrics, family medicine, or chronic care management. 2+ years of leadership or supervisory experience within a healthcare or care management setting. Strong skills in Excel, performance tracking, and EHR systems. Tech-savvy, adaptable, and passionate about improving healthcare through innovation. We are seeking an experienced and motivated Clinical Nurse Manager to lead and support our virtual care team in delivering exceptional patient-centered care in the fields of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and other preventative care programs. This nurse will oversee a team of dedicated Medical Assistants who support patients through virtual care programs while managing a small caseload of your own patients and handling clinical escalations.
This is a key leadership position blending clinical expertise, operational management, and people leadership. If you love improving care delivery, mentoring staff, and using technology to make healthcare more accessible, this role is for you.
