• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.\n
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed\n
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.\n
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.\n
• Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.\n
• Provide care to the most upmost standard\n\nMinimum Qualifications : \n
• New York State RN license\n
• Minimum of one year of recent hands on experience as a Registered Nurse.\n
• Basic Life Support Certification\n
• Valid New York State Driver's License and reliable transportation\n\nSalary Range\n\nUSD $65.00 - USD $72.00 / Hr.\n\nThis range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
