Mindful Care is seeking a Care Manager for our West Hempstead location where you'll play a pivotal role in providing exceptional patient service by fostering effective communication and demonstrating a strong understanding of the services we offer. As a Care Manager, you'll work closely with clinical staff to ensure a comprehensive and patient-centered approach to care.
Our Care Managers :
- Conduct thorough assessments of patient concerns and expectations, ensuring alignment with Mindful Care processes to enhance patient satisfaction and compliance.
- Maintain effective communication with patients, pharmacies, referral sources, hospitals, and therapists to meet patient needs and optimize retention.
- Support patients in obtaining medical records, completing necessary forms, managing prescription refills, and accessing GeneSight testing results.
- Secure prior authorizations from insurance companies for medications and services, including Transcranial Magnetic Stimulation (TMS) when applicable.
- Connect patients to the appropriate departments for their inquiries and anticipate the need for emergency interventions when necessary.
- Provide referral information to patients upon discharge from Mindful Care.
- Serve as a liaison between Mindful Care Patient Coordinators, Patient Associates, and psychiatric / therapeutic providers.
- Meet with patients for care management appointments as assigned by providers, addressing patient needs and concerns.
- Document clinical notes for care management and digital evaluations (e.g., medication concerns) and ensure timely submission for provider review and signature within 72 hours.
- Assist with TMS operations when applicable, based on office needs.
- Perform administrative duties and other tasks as assigned by management.
Qualifications
Required : 3+ years of experience in administrative functions or case management.Preferred : Possession of BLS or CPR / First Aid certification.Preferred : Experience with Transcranial Magnetic Stimulation (TMS) administration.Required : Proficiency with Google Suite and Microsoft Office (Word, Excel, Outlook, Teams).Required : Associate degree required in social work, psychology or related field; bachelor's degree preferred (experience may substitute for a bachelor's degree).Required : Excellent verbal and written communication skills.Required : Reliable transportation and home internet access.Position & Schedule
