We are seeking a dedicated professional to fill a dual role : providing Outreach support to individuals in ICF / IID facilities and assisting with Quality Assurance to ensure accurate Department of Human Services (DHS) documentation, as directed by the Division of Developmental Disabilities (DDD).

Responsibilities

Under the direct supervision of the Regional Coordinator of Prairieland Service Coordination, Inc., the Outreach & Quality Assurance staff provide education and outreach to individuals residing in ICF / IID facilities within their assigned region. This role educates individuals and their guardians on available services, completes referrals, and supports informed choice through outreach visits, written correspondence, and follow-up activities as directed by the Division of Developmental Disabilities (DDD). In addition, the position provides quality assurance support by working closely with Individual Service Coordinators (ISCs) and Program Managers to prepare and review Department of Human Services (DHS) required documents, ensure accuracy in data system, and update databases as needed.

Qualifications :

• High School diploma required; college coursework in Data Entry, Office Administration, or related fields preferred.
• 5+ years of clerical / administrative experience; proficient in typing, data entry, and Microsoft Office.
• Strong communication and organizational skills; ability to work independently.
