Licensed Practical Nurse

Job Classification : Clinical

Job Summary :

To provide prescribed nursing care services to the ill, injured, convalescent and disabled persons in the home care setting by following a Plan of Care (POC) according to the patients needs. This includes comprehensive health and psychological evaluation, monitoring of the condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.

Essential Functions :

Must use the ability to transfer scientific knowledge from social, biological and physical sciences in the application of the nursing process.

Responsibilities :

Must demonstrate knowledge and competency in performing baseline nursing skills as outlines in agency policies and clinical policy; maintains clinical competencies according to home care standards; complies with state practice act. Is responsible and accountable for making decisions that are based on the individual’s nursing experience and educational preparation.

Implements nursing care to patients in the home setting. Works under the direct supervision of an RN / MD. Administrators all medications and treatments according to Agency policy, accepted standards of care and home care guidelines makes readily visits as scheduled and participates in on-call rotation as required.

Perform selected skills actions in the provision of curative, rehabilitative, palliative or preventative nursing care. Assist with rehabilitation according to the medical care plan by encouraging patients to help themselves using their own capabilities. Report abnormalities in the patients range of motion, body mechanics or body alignment.

Identifies and determines priority of the patients needs / problems, anticipates emergencies, complications of pathology and assists others in implementing complex care. Communicates effectively with the RN, patients, families and physicians.

Documents patient status in an accurate and concise manner as required by agency policy; addresses patients needs and problems, nursing interventions and patient response, safety interventions and education and teaching. Provide a brief summary of the work performed and progress made (not to include admission). Submit clinical notes no later than 48 hours after visit.

  • Assist the Registered Nurse in carrying out the Plan of Care.
  • Assist the Registered Nurse in performing specialized procedures as directed by the physician’s orders.
  • Assist with the preparation, implementation and continuing evaluation of the patients care plan.
  • Recognize and understand the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients.
  • Help teach the patient the appropriate self-care techniques.
  • Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.

Observe, record, and report to the appropriate person patient symptoms, reactions and changes including :

  • The general physical and mental condition of patients and signs and symptoms which may be indicative of untoward changes.
  • Stress in human relationships between the patients and staff personnel and between patients and their families and visitors.
  • Maintains a committed and cooperative attitude with staffing, promoting teamwork and harmony by readily assisting other staff and patients; participates in case conference / team meetings.

    Additional Responsibilities :

  • Conforms to all applicable Agency policies and procedures.
  • Performs related duties as assigned or requested.
  • Continually strive to improve his / her nursing care by attending in-service education, through formal education, attendance at workshops, conferences, goal setting, active participation in professional and related organization and individual research.
  • Participation in the development and periodic revision of the physician’s Plan of Treatment and process change orders as needed.
  • Participate in the patient’s discharge planning.
  • Assumes accountability for reporting incidents and complaint according to Agency policy.
  • Treatment and Procedures :

    The Licensed Practical Nurse with the education and proven clinical competence may perform certain treatments / procedures provided the Home Health Agency’s policies and the State’s Regulatory Acts allow their performance. Only advance LPN’s with documented skill in performing these functions may :

  • Administer medications
  • Hang IV’s premixed with medications
  • Start peripheral IV’s
  • Flush peripheral lines with saline or heparin solutions
  • Inspect insertion sites, change dressings, and remove peripheral IV needles / catheters
  • Calculate and adjust flow rates
  • Check blood with registered nurse
  • Necessities :

  • Must pass a criminal background check.
  • Must pass a drug test.
  • Must have the ability to fill out and turn in a time sheet in a timely manner.
  • Must have respect and take care of company technology placed in your care.
  • Knowledge / Skills / Abilities :

  • Advanced written and verbal communication skills.
  • Organizational and time management skills.
  • Ability to supervise in accordance with agency policies and applicable laws.
  • Ability to respond to common inquires or complaints, regulatory agencies or members of the business community.
  • Have friendly and cooperative attitude with patients, family and staff.
  • Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff.
  • Ability to use critical thinking to resolve problems.
  • Technology / Equipment Operation :

    Must be able to interact with a computer to check email, assess training, and look at your schedule.

    Must be able to interact with an iPad for onsite charting and documentation.

    Instruments and Equipment used on a regular basis :

    Thermometer, blood pressure cuff, glucometer, PT / INR machine, PICC line, wound vac, IV skills competency will be evaluated upon hire.

    Job Conditions :

    Must have a driver’s license and be willing to drive to patient’s residence within 50-mile radius of the current TruHeart Office (2738 East 51st St, Ste 240 Tulsa, OK 74105).

    The ability to communicate both verbally and in writing is required as frequent communication by telephone and in writing is involved.

    The ability to access a patient’s home, which may not be routinely wheelchair accessible, is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and top perform patient care.

    Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.

    Environments may be less than ideal when it comes to cleanliness, comfort, or harmony. Be prepared to work in all kinds of surroundings with varying individuals.

    Information Management :

    Has access to all patient medical records which may be discussed with the supervising nurse. Treats all information and data within the scope of the position with appropriate confidentiality and security regarding patients and business trade practices. Maintains patient rights and dignity through HIPPA compliance.

    Age Related Competencies :

    Demonstrates the basic knowledge and skills necessary to identify age-specific patient needs and appropriate for this position.

    Risk Management :

    Cooperates fully in all risk management activities and investigations.

    Keep abreast of changes in health care law.

    Maintains agency / program compliance with local, state and federal laws as well as state accreditation standards.

    Minimum Position Qualifications :

  • Education : Graduate of an accredited school of nursing for LPN
  • Experience : One or more years in home health agency or in hospital setting
  • Training : Competency assessments must be completed
  • License / Certification : Current Oklahoma LPN license, CPR, drivers license & auto insurance

    Work Attire : Scrubs

    Environmental Conditions :

    Moderate noise level; inherent potential for exposure to bloodborne pathogens, moderate stress and emotional demands.
