  • On-site locations : Hastings MN and Stillwater MN.

Are you an LPN looking to step into a leadership role in a supportive, community-focused environment? We’re hiring a Residential Coordinator / Program Manager to oversee care and daily operations at both of our assisted living homes in Hastings and Stillwater, MN .

In this role, you’ll be supported by site managers and guided by the Clinical Nurse Director and Executive Director, ensuring you have the resources and leadership you need to succeed.

This role is ideal for a licensed nurse who wants a meaningful career — combining hands-on care with team leadership and resident advocacy.

What You’ll Do :

  • Coordinate and oversee care for residents in a small assisted living setting.
  • Supervise and support staff, providing guidance, training, and performance evaluations.
  • Communicate with families, RNs, and providers regarding resident needs.
  • Maintain compliance with state and federal care regulations.
  • Participate in care planning and conduct regular follow-ups.
  • Provide occasional direct care and support during staff shortages.
  • Oversee the home’s cleanliness, safety, and organization.
  • Manage resident funds and basic budgeting.
  • Be part of an on-call rotation for emergencies.
  • Occasionally, transport clients to doctor visits or community activities with the use of a company vehicle.
  • Qualifications :

  • LPN license in the state of Minnesota (required).
  • 1+ year of supervisory or care coordination experience in assisted living or a related setting - preferred.
  • Strong communication and leadership skills.
  • Comfortable with basic computer systems (for documentation and scheduling).
  • Reliable, organized, and compassionate.
  • Ability to handle complex family dynamics with professionalism.

  • Schedule :

  • Monday-Friday, 9 AM-5 PM; occasional flexibility required to meet the needs of the group homes. This position is also responsible for occasional on-call coverage to support the sites on evenings and / or weekends.
