Responsibilities Include But Are Not Limited To :

• Under the supervision of a Registered Nurse and / or licensed Therapist, receives daily patient assignments and carries out assignments in accordance with individualized patient plan of care.
• Collects patient and family data and reports findings to RN and / or licensed Therapist in a timely manner.
• Assists in the communication of information to assigned patients.
• Under the supervision of an RN and / or licensed Therapist, performs interventions and procedures as directed to ensure the safe effective quality of care for patients.
• Reports to RN and / or licensed Therapist on a variety of aspects of patient and family information including.
• Advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient / familys needs.
• Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulation / exercises, etc.
• Serves as Hospice Aide as directed.
• Serves as Personal Care Attendant as directed.
• Proactively reports changes in skin conditions to RN, Case Manager or program manager in a timely manner.
