Job Duties
The Case Manager position is responsible for the day to day operations & growth of the HCBS (245D & PCA) services including monitoring service delivery, quality & compliance. The Case Manager provides training, supervision, and evaluation of their 245D & PCA staff and evaluation of the effectiveness of services.
Responsibilities of the role include the following but not limited to :
~ Oversee operational and administrative functions necessary to achieve the goals and objectives of Access North’s in-home supports for 245D & PCA consumers.
~ Networking, marketing & outreach with the county, other service providers, agencies, and community organizations in the service area.
~ Responsible for supervision and training as it relates to DSP compliance.
~ Document and maintain accurate records showing the services provided and time spent providing services to the consumer by the DSP.
~ Maintain relationships with County social workers and waiver specialists throughout the service delivery area.
~ Work with HR to identify & fill DSP needs throughout the service area.
~ Assure DSP training relating to 245D & PCA compliance and self-directed / person centered practices.

Minimum Education Required
Bachelor's degree is preferred but not required
Minimum Experience Required
A minimum of 50 hours of education and training related to human services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under a supervised staff
