Responsibilities

Elwyn seeks a Recovery Coach to meet with newly enrolled individuals after their screening review to assist with the enrollment process and begin developing therapeutic rapport to support their addiction recovery. As part of the Integrated Team, the role assists individuals in identifying meaningful goals for their Individual Action Plan / Treatment Plan, focusing on areas such as mental health recovery education, daily living skills, recovery from addictions, relationships, vocational activities, and leisure.

DUTIES AND RESPONSIBILITIES :

Physical and psychological wellness

(1) the individual's illness / addiction

(2) the service delivery model

(3) the roles and functions of the Individual's Team members / Intensive GLE staff and how to contact them

(4) how the family will be supported. As agreed upon with individual develops communication plans with family members.
