Case Facilitator I - Child and Adolescent

A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP / LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers.

o Links, monitors, and refers clients to appropriate community resources

o Coordinates care with appropriate internal and external parties

o Ensures appropriate behavioral health services are provided

Education : High School diploma or equivalent.

Experience : Must meet OBHP requirements = Two years' experience in mental health or community services preferred.

License : Must have a valid state issued driver's license.

First shift, Full time - Monday-Friday, no holidays or weekends, 40 hrs. weekly, paid bi-weekly.

Salary based on experience.
