Job Description\n

American Traveler is seeking an experienced Hospice RN with a VT or compact RN license and at least 2 years of hospice nursing experience.

\n \n Responsibilities\n

  • Work in a home hospice setting providing care to terminally ill patients
  • Schedule is Monday through Friday, 8am to 4pm
  • Some weekend and on-call coverage is required
  • Position requires managing a caseload of hospice patients as case manager
  • Uses McKesson EMR for documentation and process compliance
  • Serve as case manager for assigned hospice patients, ensuring comprehensive care and adherence to individualized care plans
  • Coordinate and supervise LPNs, Hospice Aides, and collaborate with physicians and care team
  • Maintain up-to-date and accurate documentation in accordance with regulatory and agency standards
  • Responsible for patient and caregiver education regarding medication, treatment options, and care processes
  • Onboarding includes agency protocol training regarding documentation and infection control standards
  • Responsible for the safe coordination of patient discharges and transfers
  • Participation in peer review audits and regular team meetings is expected
  • Local candidates are accepted

  • Active VT or compact RN license required
  • Minimum of 2 years of recent hospice nursing experience
  • Competency in IV use, wound VAC, and complex wound care
  • Ability to participate in interdisciplinary team meetings and coordinate care
  • Familiarity with McKesson EMR systems preferred
