Social Worker

We’re partnering with a health plan client on a major initiative launching January 2026 . We’re seeking Michigan licensed clinicians (RNs, LCSWs, etc.) to conduct Health Risk Assessments (HRAs) for newly enrolled Medicare members.

• Timeline : HRAs must be completed within 90 days of enrollment (~12–13k members).
• Modality : Virtual / telephonic for most members; in-person for high-risk / immediate needs (~15–20%).
• Location : Detroit metro + rural Michigan (travel required for some).

This is a large-scale, high-impact project offering consistent work with one of the nation’s leading health plans.
