American Traveler is seeking an experienced RN Case Manager with recent acute care case management and Epic experience for a supportive hospital team.
Responsibilities
Work in a large teaching hospital with Trauma Level III designationPatient population includes adult medical / surgical, cardiac, orthopedic, and possibly pediatric casesDaily caseload ranges from 14 to 16 patients, depending on unit assignmentUses Epic EMR system and Rover Chat; experience with Epic is requiredPossible float to any unit within the hospitalDepartmental orientation lasts 2-5 days based on individual competence and needsWeekend rotation schedule varies by contract and all holidays are required unless approved offNo call responsibilitiesResponsible for managing complex discharge placements and collaborating with social work for at-risk and homeless populationsWork closely with a supportive team including directors, managers, UM staff, peer case managers, and social workersCareport experience is preferred for referral managementScrub color is navy blueFree parking availableSupportive and communicative department culture with seasoned staff and educatorsManager approval required for requested time offTravelers are expected to work all holidays unless otherwise approvedRequirements
Active RN license in NC or compact stateBachelor of Science in Nursing (BSN) requiredBLS certification requiredAt least 3-5 years of recent acute care case management as an RNPrior travel experience as an RN Case Manager is mandatoryExperience with Epic EMR is requiredCase management certification preferredProficiency in placement for SNF, IPR, LTAC, hospice, home health, DME, and behavioral healthCare coordination, assessment, and discharge planning skills requiredBenefits
