Job Summary
Provides training, supervision and support to individuals who experience intellectual / developmental disabilities in employment-related activities which promotes an individual's independence and success in the workplace, while focusing on integration and coordination of supported employment services.
Primary Responsibilities
- Provide comprehensive case management including accurate, complete and timely reports and statistical records on the progress of assigned individuals in compliance with internal and external regulations.
- Submit timely and accurate billing reports according to billing procedures (i.e. follow along, job coaching, and situational assessment reports; all DARS billing info is accurate, with reconciliation for bi-weekly and monthly cycles).
- Assist in direct operations as needed to ensure fulfillment of contractual obligations.
- Ensure that employees complete work to customer specifications.
- Performs related duties and responsibilities as assigned / requested.
Additional Duties and Responsibilities
Maintain familiarity with Medicaid, CARF, and funding source regulations and ensure ongoing compliance with such regulations.Provide training to and coordinate with supervisory personnel to ensure the proper implementation of vocational supports.Complete in an accurate and timely manner all evaluation / assessment reports, progress reports, case notes, incident reports, attendance and other supporting documentation in accordance with standard operating procedures.Perform quality assurance of data and attendance records to ensure accuracy and compliance with regulations.Maintain case records according to CARF standards, Medicaid regulations, NISH regulations, other funding regulations and internal policies.Coordinate assessments of assigned individuals. Plan, organize, and implement individual support plans as required.Maintain working knowledge of contract requirements.Responsible for timely and accurate records and reports necessary for the operation of department as required by worksite, including attendance sheets, case notes, productivity evaluations, safety inspections, frequency data graphs, quarterly meeting minutes and progress reports.Qualifications : Education, Experience and Certification(s)
High School diploma or GED required.Bachelor's degree in Human Services, Education, Marketing, or related field strongly preferred.One year of related experience can substitute for one year of education.Minimum one year of related human service experience required.Colorado nationally-recognized training certificate or nationally-recognized certification in Supported Employment required within 6 months of hire and maintained throughout employment.Valid driver's license and / or access to reliable transportation to perform work-related travel 50% of the time in Colorado. Eligible drivers must have a good motor vehicle record (MVR).Availability and willingness to travel to multiple locations, at times with little notice required.Availability and willingness to work flexible hours, including weekends and evenings when needed.Job development or vocational support skills required.Fluency in a second language (ASL, Spanish) preferred.Knowledge, Skills And Abilities
Proficiency in Microsoft Office skills, with the ability to become familiar with corporate-specific programs and software.Excellent oral / verbal communication skills.Safety & Health Responsibilities
