Care Coordinator / Scheduler / On Call

The Care Coordinator is responsible for ensuring exceptional client and family experiences through proactive care management, effective scheduling, and responsive communication. This role plays a key part in maintaining continuity of care and supporting caregivers and clients across our service area.

Coordinate and maintain caregiver schedules to ensure all client shifts are staffed appropriately and on time.Serve as primary scheduler, ensuring caregivers are matched to clients based on skills, preferences, and care needs.Step in to assist with coverage when call-outs occur including, when necessary, filling shifts personally to ensure continuous client care.Develop and maintain individualized client care plans that support safety, quality, and satisfaction.Communicate regularly with clients, families, caregivers, and referral sources to ensure high-quality, consistent service.Mentor and support caregivers through coaching, training, and ongoing engagement to promote retention and professional growth.Maintain a calm, professional demeanor and manage multiple priorities in a fast-paced office environment.Participate in a rotating on-call schedule to manage after-hours and weekend scheduling needs, including responding to urgent staffing or client care issues.Collaborate with the care team to ensure continuity of care, timely updates, and proactive communication.Requirements
High school diploma or equivalent; two years of business, scheduling, or healthcare-related experience preferred.Previous experience in home care, healthcare administration, or medical office scheduling strongly preferred.Strong organizational, communication, and problem-solving skills.Ability to work independently, exercise sound judgment, and remain calm under pressure.Must have a valid drivers license, reliable transportation, and current automobile insurance.Availability to participate in rotating on-call scheduling coverage (evenings and weekends).Employment Type
