- Conduct initial assessments and develop personalized care plans for clients
- Administer medications and treatments as prescribed by healthcare providers
- Monitor and document clients' progress and report any changes to the healthcare team
- Provide skilled nursing care, including wound care, catheterization, and tracheostomy care
- Assist with activities of daily living such as bathing, dressing, and grooming
- Educate clients and their families on proper care techniques and disease management
- Collaborate with interdisciplinary healthcare team to ensure holistic care for clients
- Maintain accurate and thorough documentation of all care provided
