Job Summary :
The Ambulatory Care Pharmacy Coordinator is employed by Erlanger and is responsible for overseeing and growing the pharmacy initiatives related to ambulatory care. The Ambulatory Care Pharmacy Coordinator is responsible for working directly with Pharmacy and EMG leadership to support high-impact clinical and operational initiatives. This includes ensuring regulatory compliance, policy development and alignment, hiring / supervision of ambulatory care pharmacy staff, and establishing an Ambulatory Care Residency Program (PGY1). This role will work with senior leadership to develop, implement, sustain and grow new and existing pharmacy programs and services. They will conduct analyses using relevant data and other tools to determine the impact of pharmacy program decisions and to implement business and / or operational components of the pharmacy program. They will also work closely with the Quality Improvement team to improve quality metrics across the entire EMG service line, overseeing the transitions of care team and leveraging clinical services to improve access to medications and the quality of care provided to our patients.
This pharmacist will serve as a liaison between the Department of Pharmacy and Erlanger Medical Group and will interact with a wide variety of stakeholders throughout the organization.
Education : Required :
Minimum of a Doctorate of Pharmacy Degree from an accredited School of Pharmacy.
Post-Graduate training from an ASHP Accredited Residency or Fellowship
Preferred : Board Certification
Masters in Business Administration or Health Administration
Experience : Required :
Knowledge of general pharmacy practice and pharmaceutical care with emphasis on ambulatory care
Knowledge of current trends and new developments in ambulatory care
Ability to develop strategy, identify and track performance on key indicators and cost indicators, and analyze performance against national benchmarks.
Development of financial projections and ROI models for commencement and expansion of a pharmacy program
Organized and detail-oriented; consistently meets timelines and objectives
Mature professional seen as a mature team member, interacts with internal and external stakeholders independently and in a professional manner

Department Position Summary :
The Ambulatory Care Pharmacy Coordinator employed by Erlanger is responsible for providing and embracing pharmaceutical care for all patients. This is accomplished through the oversight of the ambulatory care pharmacy staff and development of ambulatory care pharmacy service lines. This includes, but is not limited to, maintaining compliance to DNV standards and the Board of Pharmacy.
The pharmacy coordinator will collaborate with pharmacy managers at other campuses, lead pharmacists at other campuses, the Pharmacy Operations Manager, Director of Pharmacy, and the Senior Director of Pharmacy to standardize processes.
The Ambulatory Care Pharmacy Coordinator Serves As The Liaison Between Their Respective Department And Nursing, Physicians, Ancillary Departments And Leadership. This Includes, But Is Not Limited To :
Medication Storage and Waste Oversight : providing education and ensuring compliance with policy for proper medication storage, use and disposal.
Medication Safety Oversight : focusing / promoting medication safety through operational / system changes, review / respond to any medication related occurrence, participate in med safety committee(s), Apparent Cause Analyses (ACA's) or Root Cause Analyses (RCA's) and provide education and training
Medication Management : collaborate with pharmacy team and other health care providers in medication management for EMG; serve on EMG committees, ad hoc committees / focus groups, etc.
Strategic Planning : participates / collaborates with Senior Director of Pharmacy / EMG leadership for departmental strategic planning including :
Improving clinical management and quality indicators
Initiating and managing population health initiatives
Establishing and implementing strategies to reduce re-admission rates alongside the Quality Transitions of Care team
Eliminating hurdles to medication access where feasible and sustainable
Increasing outpatient charge capture and pharmacy service revenue
