Job Summary

Carease Health is looking for an experienced and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to provide high-quality, preventative care in skilled nursing facilities. This hybrid position blends in-person visits with remote work and is fully supported by a dedicated scribe team to streamline your documentation.

This role is ideal for a provider who values meaningful patient relationships, enjoys autonomy with support, and takes a collaborative approach to care. You’ll be a trusted clinical presence at your facility while benefiting from flexibility, strong compensation, and a supportive team environment.

Requirements :

Active NP or PA license (in the state of practice)

1+ year of experience in :

Geriatric care

Documentation review

Excellent communication, collaboration, and teamwork skills
