Company : Home Health Care, Inc.

Location : Southwest Minnesota

Compensation : $100,000 - $200,000 annually + $15,000 Sign-On Bonus!

Your Responsibilities :

Conduct in-home PT evaluations and craft custom treatment plans

Educate patients and caregivers on exercises, safety, and mobility

Work with interdisciplinary teams to ensure coordinated care

Document patient progress and update care strategies as needed

Requirements :

MN Physical Therapy license

Driver's license and reliable transportation

Home care or rehab experience preferred

Passion for serving patients in rural Minnesota
