We are currently hiring DSP in :
ShakopeeEvenings weekdays
Weekends rotation between morning and evening shiftNorthfieldEvening weekday shifts
Weekends rotation between morning and evening shiftAwake overnight alternating weekly schedulePay
$18.50 to $24.00 per hour
Pay rate varies based on location, shift type, and job responsibilities.
SIGN ON BONUS : $1,000 ( $500 first 6 months, additional $500 after 1 year of employment )

You will provide support that is both compassionate and compliant with 245D standards, including :
Providing direct care in line with each persons Coordinated Service and Support Plan (CSSP) and CSSP AddendumAssisting with personal cares, activities of daily living, hygiene, meals, and mobilityMaintaining client safety and supervision in accordance with 245D licensingOffering cueing, coaching, redirection, and crisis prevention supportAssisting with meal preparation, household tasks, and medication remindersTeaching and supporting independent living skills such as budgeting, cooking, cleaning, and basic health and safety routinesProviding transportation to appointments, outings, and community activities using a company vehicleSupporting community integration, relationship-building, and meaningful participation in daily lifeImplementing and tracking individual outcomes and support plan goalsFollowing Individual Abuse Prevention Plans and Positive Support StrategiesCompleting accurate daily documentation and progress notes that meet 245D requirementsReporting incidents in accordance with the Minnesota Vulnerable Adult Act and company policyCommunicating regularly with team members, supervisors, guardians, and case managersBenefits
