Hospice of the Midwest is seeking a passionate, dedicated Social Worker to join our team in Edina, MN . Our Social Worker s provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect .   • A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in
  • 418.114(b)(3).

  • Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death / dying.

  • Experience in a hospice care preferred.

  • Demonstrates good verbal and written communication, and organization skills.

  • Current licensure according to Medicare guidelines to practice in state of operation.
