Job Description

A well-established healthcare facility south of Brookline, MA is seeking a Case Management Nurse to join their team! Details listed below.

Pay range : $29.00 -$47.00 / hourly

  • Pay based on years of experience

Case Management Nurse Opportunity :

  • Full-time, Direct Hire position
  • Schedule : Monday – Friday, 8 AM – 5 PM
  • Eligible for $5,000 Sign-on bonus
  • Comprehensive Benefits Starting Day One

    • Case Management Nurse Responsibilities :

  • Case Management Nurse s chedules and arranges follow-up appointments and outpatient therapy sessions
  • Performs MUD stage progression check-ins to assess patient progress
  • Case Management Nurse c onducts patient intake in collaboration with therapy teams
  • Schedules and arranges follow-up appointments and outpatient therapy sessions

    • Case Management Nurse Qualifications :

  • Unrestricted RN License
  • 2+ years of experience in an Inpatient Rehab hospital setting required
  • Previous case management experience preferred
