About The Position

Job Title : Registered Nurse

Must-Have : Proficient in HomeCare HomeBase and OASIS

Description :

Provides coordination and direction for the provision of home nursing care according to physician orders.

Care is delivered based on agency policies and procedures through the competent application of the nursing process.

Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits are required, along with periodic office meetings.

Reliable transportation, a current driver’s license, and agency-required liability insurance are necessary.

On average, the nurse will visit 5 to 8 patients per day.

The nurse must achieve a points requirement of 30 points per week.

Different visit types and activities hold varying point values.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

It is required that the nurse has recent specialty experience.

Weekend requirements vary by location.

On-call requirements may vary per location.

All documentation must be completed within 24 hours of patient visits.

Delayed or incomplete documentation that impacts billing and follow-up care can result in cancellation of the contract.

No guaranteed hours are available in this position.

Nurse must be willing to travel up to 75 miles from the home clinic.

Mileage will be reimbursed at the IRS standard rate.

Local candidates are accepted.
