About The Position

Job Title : RN Case Manager

Certifications : Prefer certification in case management

Ability to work independently.

Ability to identify work that needs to be done and follow through to completion without prompting.

Ability to work collaboratively with the care team.

Ability to complete a comprehensive patient assessment to drive discharge.

Ability to drive stay progression and complete discharge planning efficiently.

Ability to document comprehensively, accurately, and in a timely manner.

Description :

General job duties include collaborative work with the care team.

Completion of comprehensive patient assessments, including patient status, to drive discharge.

Driving stay progression.

Completion of discharge planning to expedite patient transitions to the next level of care.

Utilization of Epic documentation for comprehensive and accurate patient records.

Equipment used includes computerized charting systems.

RN Case Managers will primarily be assigned to one unit but may need to float to another unit while performing inpatient case management work.

Units typically range from 20 to 36 beds.

Nurse to patient ratio will vary based on unit assignments.

Dress code is business casual with a white lab coat or eggplant colored scrubs.

Training involves general orientation, Epic training, and time with peers for role-specific training.

Limited assignments will occur with peer oversight.
