Job Summary

Employment Duration : 6 months

Position Requirements : Standard schedule is Monday through Friday, 8 : 00 AM - 5 : 00 PM . Occasional availability during evenings or weekends is required to support events and initiatives related to the BCH Trauma Program.

The Aftercare Coordinator serves as a dedicated Patient Navigator within UCSF Benioff Children's Hospital Oakland's Trauma Program, leading the implementation of a hub-and-spoke model to support children and families following traumatic brain injury (TBI).

This role bridges the gap between inpatient care and long-term recovery, guiding patients through post-acute services including neuro-recovery clinics, school reintegration, and community-based resources.

As a central point of contact, the Aftercare Coordinator ensures continuity of care and empowers families with education, advocacy, and resource navigation. The position also co-leads the hospital's Trauma Survivorship Program, with a specialized focus on TBI.

Key Responsibilities :

Patient Navigation & Family Support

  • Serve as the primary liaison for families throughout the TBI recovery process.
  • Provide emotional support, education, and tailored resource navigation.
  • Facilitate care transitions between inpatient, outpatient, and community services.
  • Support school reintegration and long-term adaptation to daily life post-TBI.

Program Development & Coordination

  • Design and implement program components in collaboration with trauma leadership.
  • Develop and maintain collaborative partnerships across UCSF and external organizations.
  • Coordinate multidisciplinary team efforts to ensure integrated care pathways.
  • Contribute to trauma program strategy with a focus on survivorship and recovery outcomes.
  • Project Management & Administrative Support

  • Develop and manage project plans, charters, budgets, and status reports.
  • Track deliverables, risks, resource needs, and interdependencies across workstreams.
  • Prepare presentation materials, facilitate meetings, and document outcomes.
  • Maintain confidentiality and integrity in all aspects of project documentation.
  • Education & Training

  • Assist in creating educational curricula for staff and families.
  • Deliver trainings to internal stakeholders and external partners.
  • Act as a content expert in TBI recovery and survivorship navigation.
  • Quality Improvement & Community Engagement

  • Participate in trauma-focused process improvement initiatives.
  • Support community events during evenings and weekends as needed.
  • Lead or contribute to data collection and outcome reporting activities.

    Department Description

    UCSF's Temporary Employment Program (TEP) recruits and hires temporary employees for immediate clerical and technical support services to UCSF Departments and various off-campus locations.     Required Qualifications

  • Bachelor's degree in a related area and / or equivalent experience or training (e.g., health education, healthcare field, or equivalent).
  • Experience in patient navigation, care coordination, or a closely related role with direct patient interaction.
  • Strong ability to assess patient needs, provide education, and connect patients with appropriate clinical and community resources.
  • Excellent communication and interpersonal skills, with proven success supporting patients through sensitive or complex situations.
  • Strong customer service mindset and ability to work effectively in high-volume, fast-paced environments.
  • Skilled at problem-solving, prioritization, and working across multiple teams or departments.
  • Preferred Qualifications

  • Background in healthcare or social services, such as :
  • Licensed Clinical Social Worker (LCSW)

  • Registered Nurse (RN)
  • Master's in Public Health (MPH)
  • Or equivalent healthcare experience with significant patient-facing responsibility.
  • Knowledge across multiple clinical areas or deep expertise in a specialty.
  • Familiarity with Microsoft Office (Word, Excel, Outlook) and comfort with clinical / administrative software tools.
