• Monday through Friday, 8am - 4 : 30pm schedule with 40-hour guarantee
• Home health setting providing physical therapy services
• Rotating weekend and holiday coverage when needed no call
• Comprehensive patient care including care plan development
• Experience with rehabilitation equipment and modalities required
• EMR : OASIS
• 2+ years acute care and home health experience required
• Team collaboration in coordinated care delivery
