The Dietitian will perform approximately 40 assessments per month, working closely with patients to promote nutrition education and support health outcomes. The focus will be on food security, diabetes management, and obesity prevention, with a strong emphasis on practical, personalized meal planning and lifestyle guidance.
Key Responsibilities
- Conduct comprehensive nutritional assessments for patients in both clinic and home environments
- Develop and implement individualized care plans addressing dietary needs and health goals
- Provide education and counseling on food security, diabetes, obesity, and chronic disease prevention
- Collaborate with interdisciplinary healthcare professionals to ensure coordinated care
- Maintain accurate and timely documentation of assessments and progress notes
Qualifications
Prior experience in clinical or community nutrition strongly preferredStrong communication and patient education skillsSelf-motivated, organized, and able to manage time effectively in a flexible settingBachelor’s or higher degree in Dietetics, Nutrition, or related field (ACEND-accredited). Must hold current RD / RDN credential and Iowa state licenseContract Details
