Come join our team as we partner with a world-renowned hospital on an exciting and innovative Hospital at Home program. We're redefining patient care by bringing hospital-level services directly into the home—bridging the gap between acute inpatient care and home health.

We are currently seeking a compassionate and skilled Registered Nurse (RN) with experience in Med-Surg, PCU, Telemetry, ER, or ICU to deliver high-acuity care in the home setting. This is a unique opportunity to be at the forefront of a groundbreaking care model that prioritizes patient comfort, continuity of care, and better outcomes.  Shift types are 6 : 00 – 14 : 00, 8 : 00 – 20 : 00, 12 : 00 - 24 : 00 and 15 : 00 – 23 : 00.

Responsibilities

• Deliver compassionate, high-quality nursing care in patients’ homes
• Perform comprehensive assessments
• Administer medications, treatments, and procedures
• Monitor patient conditions closely, recognizing early signs of complications and taking proactive steps to prevent deterioration
• Collaborate with physicians, therapists, and the broader care team to coordinate care and optimize outcomes
• Educate patients and their families on disease management, medication use, and self-care
• Use telemedicine tools and remote monitoring technology
