Degree from an accredited Physical Therapy programCurrent state license and Basic Life Support (BLS) certificationMinimum 1 year of Physical Therapy experience in the last 36 monthsProficiency with Electronic Health RecordsProof of required immunizations and recent PPD testStrong communication skills in EnglishMust meet all credentialing and background checksResponsibilities
Evaluate and provide rehabilitation services for patients with a wide range of conditions including post-stroke, arthritis, post-surgical recovery, and chronic painDevelop and implement patient-centered treatment plansProvide education and written home exercise programsDocument care in the Electronic Health Record systemCollaborate with physicians and participate in patient-centered medical home modelMaintain professional standards, cultural sensitivity, and patient confidentialityParticipate in committees, staff development, and peer reviews as assignedTo Apply
