The unique responsibilities for this role include but are not limited to the essential functions listed as follows : \n\nResident Care Duties\n

• Deliver care and services for designated group of guests / residents according to Individualized Service Plans (ISP).\n
• Report all incidents immediately to appropriate supervisor / team member.\n
• Report suspected abuse immediately per regulations and Sunrise standards.\n
• Participate in the care planning for their designated guests / residents\n
• Perform. individualized personal care and activities of daily living (ADL); documents ADL in flow sheets\n
• Partner with licensed nurse to plan, develop, organize, provide and execute individualized restorative therapy programs, activities and standards for their designated guests / residents.\n
• Obtain, document and report the guest / resident's vital signs, urine output and meal intake per individualized care plan to licensed nurse as requested.\n
• Observe and report any physical and / or psychosocial changes of condition requiring a licensed nurse intervention and care services.\n
• Assist in maintaining assistive devices, adaptive equipment and other medical equipment to include cleaning equipment and making readily available.\n
• Maintain Restorative Records for each guests / residents.\n
• Maintains knowledge of federal, state / provincial and local resident care regulations\n\nQuality Indicators\n
• Strive for excellent quality care and service delivery as measured in the Quality Assurance and Performance Improvement (QAPI) process.\n
• Competent of behavior interventions and management.\n
• Demonstrate and knowledgeable of the following key quality improvement areas of the DNA / C.N.A. role : \n
• ADL, skin care, dining assistance, bowel / bladder elimination\n
• Falls and fall prevention\n
• Skilled nursing transformation and Resident Centered Care model\n\nGuest / Resident, Family and Visitor Relations\n
• Respond to guest / resident, family, visitor's inquiries, concerns in a timely and professional manner using the Problem Resolution process.\n
• Invite, Encourage and Assist (IEA) guests / residents to meals.\n
• Assist guests / residents when transferring or discharged by collecting belongings, escorting to transportation, other as assigned.\n
• Greet new guests / residents and escort to their room; orients guests / residents to room / call lights and community layout and provide activity calendar.\n
• Inventory guest / resident's belongings and labels at time of admission and ongoing there after.\n\nRisk Management and General Safety\n
• Partner with community team to ensure community is in compliance with national / provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.\n
• Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS) and Lockout Tagout procedures.\n
• Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous materials.\n
• Report occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.\n
• Ensure oxygen tanks are stored safely, exchanges guest / residents tanks when empty and monitors to make sure liters of oxygen are at prescribed levels\n\nTraining, and Contributing to Team Success\n
• Participate actively as a member of a team and committed to working toward team goals.\n

• Contribute in the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.\n
• Attend regular meetings; Town Hall, Department Team Meetings, Cross Over, and others as directed by the Supervisor / Department Coordinator.\n
• Participate in annual in-service as set forth by federal and state / provincial regulations.\n
• Maintain compliance in assigned required training and all training required by state / province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.\n

• Maintains a valid license certificate as a Certified Nursing Assistant in accordance with federal, state / province and local regulations\n
• High School Degree / GED required\n
• Previous experience as a Certified Nursing Assistant preferred\n
• Previous experience and desire to serve and care for seniors\n
• Ability to handle multiple priorities\n
• Possess written and verbal skills for effective communication and documentation\n
• Competent in organizational, time management skills\n
• Demonstrates good judgment, problem solving and decision making skills\n
• Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times\n
• Ability to react and remain calm in difficult situations and be considerate\n
