Experience in a residential program preferred. Required to provide coverage when offered (within your identified availability), respond to staff when contacted to request coverage within 48 hours, and report to shifts once commitment is made. Must provide coverage at a minimum, 1 shift per month. Valid driver's license, safe driving record, valid auto insurance and access to a vehicle is required. Overnight & Weekend Differential Pay may be applicable. Port Jefferson Station Medford Oakdale East Islip Huntington Station Ronkonkoma Participate in the planning and evaluation of individual service plans and monitor plan objectives. Train and assist residents in the activities of daily living. Document services provided with appropriate notes. Manage medication and document accordingly. Provide case management and follow-up service. Provide counseling, behavioral intervention and crisis management services while following emergency procedures. Assist in the day-to-day management and household maintenance of the community residence in a professional manner. Provide transportation to residents as needed. Responsible for the management of residence in the absence of a full time Residential Counselor. Notify the Program Supervisor / Program Manager of any medical or psychiatric emergency. Complete overnight checklists and after-hours log during overnight shifts. Read and enter notes in Foothold Communication Log every shift. Complete and maintain all training requirements in a timely manner. Other relevant responsibilities as assigned by management.

Schedule : Varying shifts including weekday, weeknight, overnight and weekends, depending on shift availability
