Primary Care Physician (PACE Program) Schedule :

What You’ll Do Serve as the primary care provider for a dedicated panel of seniors Deliver direct medical care in diverse settings :
at the PACE center, in participants’ homes, and in nursing facilities Order and manage medications, labs, diagnostics, and treatment plans Collaborate with an interdisciplinary care team to design and execute individualized care plans Support transitions through hospitalizations and post-acute care Perform routine in-office procedures and consult on specialty referrals Educate participants and caregivers about preventive care and chronic disease management Keep accurate, detailed, and timely EMR documentation Participate in on-call rotation for after-hours clinical support Contribute to organizational growth and value-based care initiatives You Bring MD or DO, board-certified in Internal Medicine, Family Medicine, or Geriatrics Current state medical license and DEA Experience working with elderly populations preferred Fluent or conversational in Spanish highly preferred Knowledge of interdisciplinary care and managed care models is a plus Strong communication, teamwork, and empathy If you’re looking to blend clinical care with purpose , and want to be part of a team that's redefining aging through compassionate innovation— apply now .Powered by JazzHR
