Job Title : Licensed Practical Nurse - LPN Visit Nurse

Compensation : $30 - $38 per visit

Benefits : Full Benefits Package

Location : Twin Cities Metro Area

Schedule : Full-Time or Part-Time Availability

Position Summary : Home Care is in high demand and we are looking for experienced and caring LPNs to join our team at Home Health Care Incorporated! Successful applicants will have a proven track record as an LPN, with home care experience a huge benefit but not required. We encourage you to apply today!

Responsibilities :

Provide patient care according to client's care plan and needs

Perform wound care treatments and assist clients with medication management

Teach the client and / or family members about care and support skills

Communicate regularly with RN Case Managers, Supervisors, and the Care Team

Qualifications :

Licensed as a Practical Nurse / LPN in the state of Minnesota

Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License

Demonstrated ability to work autonomously

Prior Nursing experience in a TCU, Hospice, or Home Health setting

Experience with Electronic Medical Records (EMR) software is preferred
