Schedule : PRN – Minimum 3 shifts per month – Monday to Friday – 8 a.m. to 4 : 30 p.m.

As a Physical Therapist, you will :

• Deliver Direct Client Care – Conduct comprehensive evaluations and develop personalized treatment plans with short-term and long-term goals.
• Maintain Accurate Client Records – Document services accurately and timely using Vivie’s EMR system, ensuring compliance with all payer sources and authorization requirements.
• Facilitate Follow-Up Programs – Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes.
• Collaborate and Communicate Effectively – Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
• Ensure Regulatory Compliance – Complete and document supervisory visits with PTAs and RA / CNAs in accordance with state and regulatory guidelines.
