This position works collaboratively with physicians, nurses, and an interdisciplinary care team to assess, plan, and implement individualized care plans that promote optimal health outcomes.\n\nTypical hours are 4 Shifts : 8 AM to 4 : 30 PM. 1 Shift : 10 AM - 6 : 30 PM\n\nRotation of Weekends & Holidays. • Manage a panel of patients based on geographic location, ensuring continuity and coordination of care.\n

• Practice at the top of the nursing license, delivering timely interventions at the point of contact to prevent delays in care.\n
