Summary :
Will work with Veterans and their families to assess eligibility for participation in Supportive Services for Veteran Families (SSVF) program based on needs and barriers. Outreach Specialists will transfer eligible Veterans to Case Managers for full determination of eligibility. Outreach Specialist will travel throughout the County fostering relationships with agencies and the literal homeless to establish referrals. Saturday availability is required. Must have valid driver license with personal auto insurance as well as pass background and driver record check.
Education and / or Experience :
Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and / or training; or equivalent combination of education and experience.
Essential Duties and Responsibilities include the following :
- Provide excellent customer service.
- Perform outreach in shelters, food banks, and various local community partners as well as street level outreach to educate and refer Veterans to the SSVF program.
- Screen participants for eligibility according to program guidelines, to include using the Homeless Prevention Eligibility Screening Disposition Form.
- Provide community resources.
- Interpret and explain information, such as eligibility requirements, application details, and program specific information.
- Support program to locate landlords willing to lease to participants with housing barriers i.e. criminal convictions, poor credit, or previous eviction history, as requested.
- Stay informed of all state and federal laws affecting the program, organization and the people it serves.
- Educate the community about the SSVF grant and its resources for Veterans through community presentations and meetings.
- Create and utilize informal community networks, as needed, to locate resources adn make referrals as necessary in support of increasing veteran participation in the SSVF program.
- Inform Program Manager of any unusual events that occur and document appropriately for incident.
- Demonstrate respect and value all Veterans, regardless of circumstances, behavior or needs.
- Make necessary notifications when required to law enforcement, supervisors and other appropriate individuals.
- Ensure all paperwork and related activities are current, accurate, and in compliance in a timely manner. Includes all case notes and reports required for billing.
- Maintain appropriate boundaries with all Veterans per Goodwill's policy.
- Attend all staff meetings, in-service trainings and supervision meetings.
- Maintain flexibility to work occasional evenings or weekends shifts.
- Provide services in accordance with Goodwill policies, procedures, directives, CARF standards, and principles of safety of personnel and property.
- Facilitate Person Centered Planning to determine goals, recommend appropriate services to achieve goals, and oversee the program goals agreed upon. Revise as necessary.
- Work independently & collaboratively to provide services.
- Follow all policies, procedures, and directives of Goodwill assuring safety of personnel or property.
- Work collaboratively with department to provide mission services.
- Properly wear and maintain all required Personal Protective Equipment (PPE).
- Maintain agency confidentiality. Violation of confidentiality is cause for immediate dismissal.
- Comply with all health, safety and fire standards and all local, state and federal regulations (WISHA & OSHA).
- Other duties as assigned.
