Care Navigator

Function :

• The Care Navigator is a central point of contact for individuals accessing behavioral health, primary care, and community support services through Ramey-Estep / Re-group.
• This hybrid role combines in-person engagement with remote telehealth and administrative support to ensure clients receive timely, person-centered care.
• The Care Navigator works closely with clinicians, case managers, and community partners to reduce barriers, coordinate services, and promote recovery, wellness, and long-term stability in alignment with Certified Community Behavioral Health Clinic (CCBHC) standards.
