We are seeking a  Mobile Psych Rehabilitation Specialist   to provide skill / resource development, counseling and encouragement to clients as they move into vocational or community based activities of their choice; ensures that all clients receive the level of and / or have access to the services / supports they choose, and to monitor and track client progress of assigned caseload; participates in the development of individualized rehabilitation plans.

This position receives general supervision from designated Manager and / or Mobile Psychiatric Rehabilitation Team Leader and some of their duties include :

• Maintaining a close liaison with the local community and area service providers / agencies to facilitate program cooperation and provide for constructive input and / or alternative resources to current plans.
• Maintaining and monitoring an assigned caseload of clients.
• Participating in relevant training to enhance professional growth and development, with an emphasis on counseling, case-management and basic living skills.
• Participating in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and / or immediate intervention during a crisis situation.
