Responsibilities
Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physicianCompleting history and physical examinations by evaluating the patient's medical condition and health historyCompleting the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility basedMaking recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice careCompleting and maintaining accurate clinical notesCommunicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family / caregiversEnsuring compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assignedQualifications
Graduate from an accredited School of Nursing and accredited Nurse Practitioner programCurrent DEA license, if applicable to state regulationsCurrent nursing license in state of practiceCurrent CPR certificationMaster’s degree with minimum of one year NP experienceOne year hospice or palliative care experience preferredAccreditation from an approved certifying body for advanced practice nursing as required by state of practiceCertification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)Commitment to clinical and documentation excellence
