Responsibilities

• Deliver safe, direct patient care through skilled medication and treatment administration.
• Observe and manage acute changes in resident conditions, providing timely updates to physicians and families.
• Document thoroughly and communicate effectively with residents, families, physicians, and team members.
• Supervise and mentor CNA staff, fostering a collaborative, supportive work environment.
• Attend staff meetings, report to the Director of Nursing (DON), and follow all Care & Rehab policies.
• Create a safe, comfortable home setting for residents.
