Community Health Worker

A company is looking for a Community Health Worker.

Key Responsibilities

Conduct patient outreach and resource navigation through home visits and intake interviews

Provide health education and informal counseling on chronic diseases and self-management

Develop relationships with community resources and assist in coordinating access to services

Required Qualifications

High School Diploma or G.E.D

Two years of experience in a related area

Ability to navigate city driving and hold a valid driver's license

Spanish-speaking skills are a plus

Experience working in community health settings preferred
