The Home Health and Hospice CNA provides personal care and related services in the home that reflect the philosophy and standards of home care of the home health and hospice agency, under the supervision of a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist. The Home Health & Hospice CNA provides personal care and related services in the home or other appropriate dwelling, to home health and hospice patients as designated in their plan of care.
I. Job Relationships
Reports to : Home Health & Hospice Director
Interrelationships : Patients and their families, Providers, nurses, CNAs, therapists, support staff, administrative staff, and other members of the health care team.
II. Access to Protected Health Information (PHI)
A CNA must have access to the clinical care plan information of patients to whom he / she is providing services. The CNA must have access to protected health information to perform quality improvement activities when serving on the QAPI Committee.
III. Essential Job Duties :
- Provide patient comfort and hygiene.
- Ensure quality and safe delivery of home care services.
- Perform technical skills and procedures as ordered by physicians and supervised by an RN.
- Communicate with co-workers, patients, family, and other ancillary departments.
- Supply responsibilities.'
- Accept responsibility for personal growth / Accountability / Time Management.
- Provide respite and / or continuous care for hospice patients.
- Post mortem care.
- Attend all required meetings and trainings.
- Other duties as assigned.

Education : High School graduate or GED, current CNA Certification, with the Division of Facility Services.Experience : Minimum of one (1) year of experience preferred; home health or hospice experience preferred.Other : Valid N.C. Driver’s License, proof of auto liability insurance, and dependable transportation.
