Description

Our Hospice S piritual Care Coordinator / Volunteer Coordinator have been called to care when they’re needed most. As a member of our hospice team, you’ll support a full range of patient services to bring comfort and dignity to our clients.

We’re seeking a dedicated and empathetic professional to serve in a dual role as Chaplain / Bereavement Support and Volunteer Coordinator .

As a Hospice Spiritual Care Coordinator / Volunteer Coordinator, you will :

Ensures that appropriate spiritual services are provided in a timely manner by either providing such to the patient / family or coordinating the provision of services with community clergy or spiritual counselors.

Provides direct spiritual services to patients / families according to their belief system and practice.

Maintains records of spiritual services utilization and related activities as directed for quality assessment and performance improvement (QAPI), hospice program development, and policies and procedures review and revision.

Acts as the hospice spiritual services community liaison by developing community contacts and offering hospice education to a variety of clergy, counselors and congregations.

Receive referral calls, complete intakes, document new client services enrollment, and schedule care professionals for hospice

Update / upload documents to patient charts

Meet / exceed customer expectations, as well as maintain customer relationships in an ongoing effort to drive higher levels of customer satisfaction

Ensure that hospice volunteers have everything up to date and turn in all timesheets

Be responsible for community outreach to recruit new volunteers

To qualify for a Hospice Spiritual Care Coordinator / Volunteer Coordinator with us :

Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.

Experience with grief counseling and working with death and dying, terminal individuals and their families or caregivers.

Hospice experience preferred.

Associate's degree or equivalent years of experience and training that includes management and oversight of multiple individuals; development and execution of recruitment, selection and training of individuals providing a variety of services; preparation and management of a budget and related expenditure; and maintenance of statistics related to hours served.

Minimum of two (2) years' experience in a health care setting. Hospice and / or volunteer experience preferred.
